Hospital Visitation Team

This group connects with people who are undergoing hospitalization for surgical procedures, illness, and the birth of a baby.

Outpatient Surgery – the goal will be to connect with people who are having outpatient surgery at least once by phone or in person before surgery and at least once by phone or in person after surgery, whichever is most feasible and/or desired by the patient.

Patients admitted to the hospital – The goal will be to connect with people admitted to the hospital at least once if their stay is less than one week and at least once per week for longer stays. Goal is to make a visit to a patient within 36 hours of learning of the hospitalization, if not critical, and as soon as possible if critical.

The hospital visitation team will communicate with the coordinator(s) and/or pastor when visits or attempted visits have been made and whether follow up is needed. Any information about discharge time frame, discharge location should be communicated.

Long-Term Care Visitation Team

We will seek to make a personal visit to residents in long term care at least once every two months and at least one personal contact (by phone, card, or letter) every month. These individuals will do visits singularly.

Home Bound Care Visitation Team

This group connects with people who are largely confined to their homes by illness or infirmity and cannot come to church. This team will make visits in pairs. We will seek to make a personal visit to homebound residents at least once every two months and at least one personal contact (by phone, card, or letter) every month.
